This lovely ring is evidence of extensive adhesions between the edge of the pupil and the lens, probably as a result of inflammation. . Keratoprosthesis is a surgical procedure where a diseased cornea is replaced with an artificial cornea. Traditionally, keratoprosthesis is recommended after a person has had a failure of one or more donor corneal transplants. More recently, a less invasive, non-penetrating artificial cornea has been developed which can be used in more routine cases of corneal blindness. While conventional cornea transplant uses donor tissue for transplant, an artificial cornea is used in the Keratoprosthesis procedure. The surgery is performed to restore vision in patients suffering from severely damaged cornea due to congenital birth defects, infections, injuries and burns. Keratoprotheses are made of clear plastic with excellent tissue tolerance and optical properties. They vary in design, size and even the implantation techniques may differ across different treatment centers. The procedure is done by ophthalmologists, often on an outpatient basis. Although many keratoprostheses have been developed, only four models are currently in commercial use: the Boston keratoprosthesis, Osteo-Odonto-Keratoprosthesis (OOKP), AlphaCor and the KeraKlear Artificial Cornea. Amazing vision improvement (20/400 to 20/25) with a scleral lens for a patient with keratoconus! ◽️ This patient presented to us for a specialty contact lens consultation. They had advanced keratoconus with a corneal curvature of 60.9D/72.2 D. You can see this on the second photo of the corneal topography (mapping). A regular cornea is about 43/45 Diopters in curvature. This means that our patients cornea has thinned and protruded significantly due to keratoconus. ◽️ This eye had a vision of 20/400, and was only able to be corrected with glasses to 20/200. ◽️ We designed a custom scleral lens for this patient that has four different (quad-specific) curvatures to fit around the sclera (white part of the eye). You can see on the first photo how beautifully the lens curves around the eye, allowing all the blood vessels to travel uninterrupted beneath the lens. (@bauschlomb) ◽️ The third photo shows the unique curves of the patients sclera, which is able to be mapped with our specialized Oculus Pentacam equipment and software. (@oculus_usa) ◽️ On the fourth photo you can see the lens and how it vaults over the thinned cornea. This lens does not touch the patients cornea which keeps the cornea healthy and avoids scarring. A carotid-cavernous sinus fistula (CCF) is an abnormal connection between an artery in your neck and the network of veins at the back of your eye. These veins at the back of your eye transport blood from your face and brain back to your heart and are located in small spaces behind your eyes called cavernous sinuses. Sometimes an abnormal channel forms between these veins and one of the internal or external carotid arteries that run up each side of your neck. This formation happens as a result of a small tear that sometimes occurs in one of the carotid arteries. If the tear occurs near the veins in the cavernous sinus, an abnormal channel may form between the artery and the network of veins, through which blood may flow. This is called a fistula. A fistula can raise the pressure in your cavernous sinuses, which may compress the cranial nerves located around the cavernous sinuses. This compression may damage the nerve function, which is to control your eye movements. These cranial nerves also allow you to experience sensation in parts of your face and head. The increased pressure caused by the fistula can also affect the veins that drain your eye. This can cause symptoms such as eye swelling and abnormal vision. Symptoms of Carotid-Cavernous Sinus Fistula Indirect CCFs tend to cause fewer, less serious symptoms. This is because of their relatively low rate of blood flow. Direct CCFs usually require more urgent attention. Fixing Imbalance and Double Vision with an Occlusion Pupil Contact Lens! ▫️ Here is a photo of a patient of ours wearing a prosthetic soft contact lenses with a black occlusion pupil. ▫️ This patient had balance issues and double vision due to one eye having much poorer vision than the other eye. This eye had endured complications from a cataract surgery (the intraocular lens fractured in the eye) and after removal of these pieces, he had a retinal detachment and reattachment surgery. ▫️ These surgeries left our patient with distorted 20/50 vision, even with glasses, in this eye. ▫️ Even after trying to enhance the glasses prescription, he only found relief by wearing an eye patch over this eye. ▫️ We discussed that a prosthetic occlusion pupil might be a better option for him cosmetically and he agreed. ▫️ We fit him with an Alden black prosthetic pupil 6.5mm in size. He was very happy with this lens. He wears his glasses over his contact lens for vision and protection. ▫️ Any out there use occlusion pupils frequently? This is a LAST resort option for me but I am happy to create them for the right patient. - @eyesplain on Instagram

Giant Papillary Conjunctivitis (GPC). Giant papillary conjunctivitis (GPC) is an allergic reaction of the eye. It occurs when one or several small round bumps (papillae) develop on the underside of the eyelid. The underside of the eyelid is also called the upper tarsal conjunctiva. These bumps develop when the upper tarsus rubs over something foreign in the eye. The term giant refers to the bumps, or papillae, being larger than 1 millimeter (mm) in size. There are two types of GPC, primary and secondary. Both are responses to allergens but have different causes. Primary GPC has two types which are: •vernal keratoconjunctivitis (VKC) •atopic keratoconjunctivitis (AKC) The two types of primary GPC have many similarities. They differ by having different primary causes and the age and manner of symptom onset.
