Miss Rivers came to see me today for her 2 week well child check so I thought it was the perfect opportunity to discuss newborn screenings. Rivers is thriving on her momma’s milk...just look at those cheeks and double chin!!! A newborn screening is a public health service done in each U.S. state that tests for a group of health disorders that might not be apparent at birth. Typically a screen is done at birth then at 2 weeks of life. Each state screening test varies so you will need to check with your specific state for more information. The second screen is done because some of these problems are not detected until the baby has been eating for a while. As seen in the picture, five drops of blood are placed on a piece of paper. A heel stick is used to obtain this sample. The paper is then sent to the state lab for testing. Time to get results can vary, but typically take 2-4 weeks. In Alabama, the newborn screen tests for things such as congenital hypothyroidism, adrenal hyperplasia, galactosemia, hemoglobinopathy, cystic fibrosis, etc. A baby also has a hearing and congenital heart defect screening done at birth (in most states) and these results will be available immediately. What happens if your child’s newborn screen is abnormal? Your healthcare provider will be given specific guidelines/recommendations by the state depending on which part of the screen is abnormal. Oftentimes a repeat newborn screen is the only recommendation and other times more involved blood work will be needed. ‘Inner War’ is a recent piece created after a friend committed suicide completely out of the blue. I was having my brain scanned for a voice study and I asked to use the MRI to capture a portrait instead of payment. The portrait shows me attempting to hide, and stop the view of my expressions. Of course the radio waves pass right through me revealing not just what’s beneath my hands but my skull and brain. I am transparent and seen with intense truth. Mental illness is on the rise, and more than ever we need to talk and support each other. We are so much more complex than we can give light to, many quietly at war with themselves. We must offer guidance and encourage sharing. Legends, good news. From the surgeon everything looks stable, no changes (size, density, shape) and nothing else any different from my last scan. Left is yesterday 29SEP, right was 4months ago 29MAY. So you can have a squizz yourselves. That right middle shadow bit in the tumor, with a small white cyst on it. My parents find out the news via Instagram too haha so Hi mum :) A sigh of relief until another scan in a few months, lets hope things keep on this same trajectory. The beauty of getting a potentially life changing scan every few months is I get a mortality foot up the ass 4 times a year so I dont get stuck in bad habits and forget to live. Often times new parents are surprised at how little it takes to feed their newborn. When they see the small amount of colostrum their body is producing in the first days after birth they will sometimes worry if that is in fact enough. Most of the time, it is! Your newborn only needs about 1-1.5tsp (5-7ml) of your colostrum in the first two days of life. By day 3 they are eating more, about 3/4oz-1oz (22ml-30ml) per feeding. More times than not your body is making enough for your newborn in those first few days. If you’re pregnant and planning to breastfeed, get informed about how your body works, what helps this process, what can hinder the process and how much your baby needs. 👶🏻👧🏽El síndrome del niño sacudido o zarandeado, conocido como shaken baby syndrome, es una forma de maltrato físico infantil que incluye la existencia de un traumatismo intracraneal que conlleva un conjunto de alteraciones clínicas y patológicas que conforman un síndrome bien definido. Normalmente, el daño cerebral que se produce conduce a un deterioro mental de intensidad variable, llegando en ocasiones a provocar lesiones y secuelas muy importantes en el sistema nervioso central. Hay que tener en cuenta, que cuando se produce una lesión cerebral severa como consecuencia de una sacudida de la masa encefálica, bien tenga como base un maltrato infantil intencionado o como causa de maniobras de reanimación en las que el niño es zarandeado de forma inconsciente, siempre va ocurrir antes de los tres primeros años de vida, y en su gran mayoría, en el transcurso del primer año. En cuanto a las personas que cometen el maltrato, por orden de frecuencia son, los padres, los compañeros sentimentales de las madres, las mujeres que cuidan a los niños en sus casas, y por último, las madres. El cuadro clínico más habitual entre un 40% y un 70% de los casos debuta con crisis epilépticas. De la misma forma suele ser frecuente: . ✔️ Irritabilidad ✔️ Letargia. ✔️ Alteraciones del tono muscular ✔️ Falta de apetito ✔️ Vómitos ✔️ Pérdida de conciencia ✔️ Incluso trastornos respiratorios Ante la sospecha es necesario posterior a la evaluación clínica la realización de una TAC de cráneo, de igual forma los estudios oftalmológicos serán imprescindibles desde los primeros momentos, para la detección precoz y seguimiento de las hemorragias retinianas. A pesar de la extensa difusión del maltrato infantil, se ha estimado que solo se identifica el 10% de los casos. September is #nicuawarenessmonth so I wanted to share about the lead up to the 3 and 4 weeks Sam and Zelda spent in the NICU. We were shocked to find out that we were having twins, but the idea of holding two babies in my arms at birth was something I dreamed of doing. Every twin pregnancy is high risk (especially with my history of needing bedrest) so we knew we had to be prepared for a likely NICU stay. At 31 weeks pregnant, I found out that I had gestational diabetes AND that Baby B (Zelda) was IUGR. So after a myriad of appointments and an eventual failed BPP scan and stress test at 32 weeks, I was sent to the hospital for a possible c-section. Things stabilized overnight, but I contracted the flu! So I spent 8 days in the hospital hoping and praying to keep these babies inside while living in isolation. Thankfully, my mom had flown in the week before so she was there to help Michael with the girls at home (who were only 21 months and 3.25 years old at the time). After my discharge, I was home for 5 days before returning back to the hospital for the real delivery. That morning both Baby A (Sam) and Baby B (Zelda) failed their BPP scans so straight to the hospital I went. Here I am meeting Sam (5lbs - top photo) and Zelda (3.5 lbs - bottom photo) for the first time, about 3.5 hours after they were born. Of course I never got the experience of holding the babies together in my arms after birth since they were immediately whisked away to be put on oxygen. I would have to wait a few weeks before getting to feel the joy of holding two newborns at once! Our time in the NICU feels like a distant memory now that it has been 2.5 years; however, I’ll never forget what it felt like to have my heart split in half with two toddlers at home and two preemies in the hospital (and then it was even harder once Sam was discharged 5 days before Zelda). Gratitude365 - Thankful for making a difference in the lives of newborns, infants, and children EVERYDAY! What do you do for a living? Simply put, I get to read brainwaves of newborns and test the hearing of children. Sounds cool 😎 right? It is so much more. At age 7, I told my mom and dad I would grow up and work with babies who are deaf, like Annie Sullivan and Hellen Keller. I began teaching myself sign language about the same age. Watched movies, learned finger spelling from a book, and traded babysitting for sign language lesson at age 11. Became an interpreter, early intervention provider for deaf and hard of hearing children, teacher for the deaf, started my own non-profit for early intervention, wrote and coordinated the newborn hearing screening law in Kansas and became a pediatric audiologist. I was asked to co-write a newborn hearing screening and diagnostics chapter in a text book. I love, live and breathe pediatric audiology. Every single day I get up and go to work and I am THANKFUL for this opportunity to serve these families. Thankful to make a difference.
