Mas não é o momento de pensarmos nos 80%. É hora de pensarmos nos 20%. Na sua avó que neste momento está com a saúde fragilizada. Na sua amiga que neste momento passa por uma quimioterapia. No seu pai que tem problemas respiratórios. Porque eu te amo, vou cuidar de nós. Se hipoteticamente duas mil pessoas se contaminarem em minha cidade, e dessas duas mil, cem precisarem de atendimento hospitalar ao mesmo tempo, vai faltar leito. Vai atingir pessoas frágeis nas filas de atendimento médico. Vai virar um caos. Pelos outros, eu vou me cuidar. Pelos outros eu vou ficar em casa, isolada se eu for um caso suspeito. Pelos outros eu vou me privar de viajar, se meu destino for de livre circulação do vírus. Vou evitar festas com grandes aglomerações. Vou lavar minhas mãos, vou evitar compartilhar o chimarrão, vou evitar abraços e até mesmo os apertos de mão. Pelos outros eu vou fazer minha parte. E quando cada um faz sua parte, todos ganham. Read on for tips I use when I look at an abdominal CT...⁣⁣ ⁣⁣ 👨🏽‍💻Coming into radiology as someone who was mainly concerned with what was going on above the diaphragm I found the world of abdominal CT fairly daunting. But by getting to grips with the anatomy of the abdomen things became a whole lot clearer ⁣⁣ ⁣⁣ 👨🏽‍💻At this level of the abdomen we get a nice demonstration of the superior mesenteric artery – we can’t see the superior mesenteric vein here but normally the vein should lie to the right of the artery at the level of the pancreas, if this is reversed then consider intestinal malrotation which can predispose to midgut volvulus⁣⁣ ⁣⁣ 👨🏽‍💻I always check to see there is a clean layer of fat surrounding the SMA – if this is infiltrated consider a malignant pathology such as a pancreatic tumour⁣⁣ ⁣⁣ 👨🏽‍💻Always follow the vessels no matter which CT it is you’re looking at, be it head, neck, chest or abdomen. I always follow the portal veins within the liver formed at the confluence by the splenic vein (which runs posterior to the pancreas) and the superior mesenteric vein which runs vertically. In patients with liver cirrhosis and pancreatitis in particular you are looking for a filling defect or occluded vessel representing thrombosis⁣⁣ ⁣⁣ 👨🏽‍💻There aren’t many retroperitoneal structures that cross the midline but the left renal vein is one of them – characteristically this passes anterior to the aorta from the left kidney to the IVC but in some people this can have a retroaortic course where it passes behind the aorta. I have seen the occasional incidental left renal vein thrombosis so it is worth checking for filling defects! This is a must in cases of renal cell carcinoma which likes to spread into the IVC via the renal veins⁣⁣ ⁣⁣ 🔻@theradiologistpage - @theradiologistpage on Instagram

Fresh IBCC chapter & cast on parenteral anticoagulants in critical care⁣ 🤓 ⁣ 🩸Lots of pharmacology here but I think the most interesting bits are about heparin resistance (when it’s impossible or difficult to achieve anticoagulation with a heparin infusion). We are rarely taught about this, but it’s reasonably common in ICU (especially in COVID pts). ⁣ ⁣ 🩸Heparin pseudo-resistance: this is a lab artefact when the PTT is artificially low due to excessive levels of fibrinogen and factor VIII. This is *dangerous* if not recognized, because the heparin drip may be cranked way too high, causing hemorrhage. Diagnostic key is to check the anti-Xa level (will be therapeutic or supra-therapeutic).⁣ ⁣ 🩸True heparin resistance may be due to pharmacologic changes in heparin metabolism or anti-thrombin deficiency.
