Keratoprosthesis is a surgical procedure where a diseased cornea is replaced with an artificial cornea. Traditionally, keratoprosthesis is recommended after a person has had a failure of one or more donor corneal transplants. More recently, a less invasive, non-penetrating artificial cornea has been developed which can be used in more routine cases of corneal blindness. While conventional cornea transplant uses donor tissue for transplant, an artificial cornea is used in the Keratoprosthesis procedure. The surgery is performed to restore vision in patients suffering from severely damaged cornea due to congenital birth defects, infections, injuries and burns. Keratoprotheses are made of clear plastic with excellent tissue tolerance and optical properties. They vary in design, size and even the implantation techniques may differ across different treatment centers. The procedure is done by ophthalmologists, often on an outpatient basis. Although many keratoprostheses have been developed, only four models are currently in commercial use: the Boston keratoprosthesis, Osteo-Odonto-Keratoprosthesis (OOKP), AlphaCor and the KeraKlear Artificial Cornea. What a day yesterday. I got to spend some time with @teagueholmes and were both injured. We came upon a parking attendant who told us that we could park over there and run over to the other side of the building. We both cant run, so I drove him over there and he hopped out of the car to do his errand. After that I went to the doctor for my 6 week checkup and he said that I could now sleep without my back brace on We both immediately started reminiscing about the time we got to ski the Grand Envers with Cédric Aubert. A beautiful picture of a cell we injected today in my lab with a fluorescent dye. This is a Broad thorny retinal ganglion cell (species: common marmoset/callithrix jacchus), a type of neuron located in the retina. These neurons help provide us with our sense of sight and are involved in a number of complex processes mediated through our visual neural circuitry. Cells like this one are involved in processes such as “visual pursuit” which guides our eyes as they focus on different or moving targets. For reference, this cell has a diameter of approximately 300 um, or about 3/10 of a mm. Even when I experienced this view for 3.5 months during the @mosaic_expedition, I could hardly believe that I was standing on a drifting sea ice with 4200 meters of ocean below. Remote Sensing Site science station, using satellite technology was set up to measure the sea ice and determine how snow layer influences the growth of the sea ice. One of the saddest cases I’ve come across recently was this 7 yo child. . . . She presented to A+E with ? Poor vision in the R eye for ‘some time’. . . . On examination- the R eye had Hand Movement vision, and a clear RAPD. The L eye appeared to have normal vision. There was evidence of Haab’s striae, and mild corneal oedema in the R eye. Both eyes had enlarged corneal diameters present since birth but not thought to be pathological by the parents. . . . Having looked at the discs I was shocked to see the extensive cupping of the R disc. . . . Her IOPs were only RE 32 and LE 22. Both eyes had megalocorneas. . . . Is immune testing necessary before #IVF? I frequently get this question. Sometimes more is not better and tests that are not proven to be useful may do more harm than good. Today is the #transferversary of this embryo that now is a healthy baby. After struggling at another fertility center, this couple had all kinds of expensive tests that are not necessary before IVF. My new patient was terrified that she had an immune problem that would prevent her from carrying a term pregnancy and that would affect her health. All kinds of interventions were recommended including blood thinners, steroids, intralipids. Take a step back. We knew why this couple had infertility and it had nothing to do with her immune system. She was otherwise normal and healthy. I suggested she see a medical immunologist who agreed these tests were not necessary or predictive of any underlying immune problem. I literally wrote in her chart that we are ignoring all the prior results. It would have been easy to accept them and let another doctor prescribe unnecessary medications. She might have even gotten pregnant in spite of them and thought they helped and were necessary for her future pregnancies too. Any way, on her first transfer at #columbiafertility she conceived with a natural cycle frozen embryo transfer and delivered a healthy baby. The black arrows in images A & B point to her fractured #eye socket (comminuted fracture of the left orbital floor). The rim of her eye socket took the brunt of the impact, sparing her a more severe injury to the eye. Using gene therapy to restore vision to people with an inherited retinal disorder is one of the most exciting achievements in eye research. The one-time treatment for Leber congenital amaurosis uses a harmless virus to deliver healthy copies of a missing gene to the retina. Some young patients have regained vision within weeks after treatment. Despite the miraculous outcomes, ophthalmologist continue to refine their technique for delivering gene therapy to children’s eyes. In the June issue of Ophthalmology Retina, Audina Berrocal, MD, and colleagues show a new technique for gene delivery in children. Here’s a partial description of their technique: After loading the AAV vector syringe onto a 38G/25G Subretinal PolyTip Cannula and purging all air bubbles, the injector was delivered into the subretinal space with a beveled tip. Beveling of the cannula tip was performed with scissors at a 45-degree angle, and the cannula was entered into the subretinal space in a bevel-up fashion. The EVA Phaco-Vitrectomy System with the viscous fluid control system was used for a single-surgeon, foot pedal-controlled linear infusion of the drug. When bleb tension and risks of macular hole formation are deemed too high, more than 1 bleb is recommended. With more than 1 extrafoveal bleb, subretinal delivery began nearer to the arcades but included the macula. In all cases, a total of 300 mL of vector was released (image shows multiple elevated subretinal blebs with a foveal detachment after injection). This 26 year old male had a motor vehicle accident and unfortunately battered his frontal, orbital and nasal bones. The frontal bone had both anterior and posterior table fractures with persistent cerebrospinal fluid discharge. The whole frontal bone had dished in and also was pushing the right eye out of globe. Fortunately his vision was intact and there was minimal frontal lobe injury. We had to intervene early so took him to the OR.
