Today we bring you a extremelly rare case: a 17-year-old male sustained a complete nose and upper lip amputation in a motor vehicle accident. The midface segment was emergently replanted. Two arteries (left dorsal nasal artery, left superior labial artery) and 1 vein (branch of the left supratrochlear artery) were anastomosed using microsurgical technique. - A lot of people have been reaching out and I’m unsure how it leaked out but I’m going to clear the air before the rumors start.... This morning I drove my z06 out to a car show to support a good cause this morning, and on my way home hit some rain unexpectedly. Hydroplaned doing about 60mph and hit a guard rail twice yielding the damage you see in the pics. Bad tires in the rear of the car were the cause of the accident so before people try to formulate rumors of racing or toying around I figured this was appropriate. This post kills me with everything I have inside me. Those who know me know I work my ass off for what I have, and my z06 was such a huge part of my life and ultimately my happiness/social life. Lawnmower injury 🔪 shown here is a partial amputation of the 1st, 2 and 3rd phalanges (toes) of the right foot. These types of injuries are quite common in trauma and typically result in amputation due to the extensive damage caused by the lawnmower blades. Initial treatment includes Betadine rinse, Tdap vaccine if 10 years, and pain management. The patient was then treated with IV antibiotics and taken to the OR where an attempt to salvage was performed with pinning each toe. Ultimately, amputation of the 1st toe was preformed. I don’t believe I’ve ever posted a selfie before, but this seems as good a time as any. When I volunteered to photograph a Trump rally and protest in Dallas, I figured I might see blood. I didn’t expect to see my own. In short, I got nailed by a rock. I didn’t see it coming or who threw it. Cops rushed to me, as did fellow media people. Before I got patched up, I learned a valuable lesson: It’s difficult to give an articulate interview to a swarm of journalists when you have blood streaming into your eyes. Once bandaged, I went back to photographing protesters and counter-protesters. The irony is not lost on me that I recently returned unscathed from covering a war zone, and that I only had to drive 10 minutes from my home in Dallas to join the ranks of journalists injured while trying to do their jobs. Many, many thanks to everyone who has expressed concern for my wellbeing. Know that I’m fine... but if anyone wants to come over and process all these photos for me, that’d be swell. Compartment syndrome is a medical emergency and it needs to be recognized early. A similar situation may occur when an IO cannula is deployed in the wrong position or dislodged during the treatment. That is why you would want to make sure your IO device is in place. There are several ways to verify IO placement: 1. Check for aspiration of bone marrow 2. Flush and watch for swelling 3. Are the medications injected via the IO taking effect on the patient? 4. If available, bedside doppler may be effective in correct position confirmation. Early signs of compartment syndrome - Severe pain, swelling and redness of the affected limb Compartment syndrome is a rare complication associated with IO access. It may occur when the cannula is dislodged from the bone or over penetrated it. Therefore, correct location must be verified by aspiration of bone marrow, saline flush and observation of extravasation signs. In the image above is the result of a fasciotomy performed on a patient who had arrived to a field hospital with compartment syndrome, after being trapped under wreckage of an earthquake and rescued. Many have asked how Bridger is doing with his healing. We are so grateful for the concerns, thoughts, and prayers. We have, in a very real and tangible way, been blessed by your positivity. The bottom right was the night of his injury. The bottom middle was after half the stitches came out the following week. The bottom left was after all the stitches were removed. The top two photos are before (right) and after (left) his first laser treatment. The laser treatment was amazing at resurfacing the raised areas and helping with the general coloring. However, it does take some time for all the redness caused by the laser to dissipate. Maxillofavial Trauma Management - Airway ———————————————————————— After we handle the immediate life threats (massive hemorrhage), we have to consider the airway. Look, listen, and feel: take a look at the injuries and look inside the mouth for blood, vomit, broken teeth, dentures, chewing tobacco, etc. If any of these are present, grab your suction and clear it out. If dismounted or without mechanical suction, grab a 60cc syringe with a cathedral tip and a put an NPA on it to create a field expedient manual suction device. Give the lungs a listen and see if you’re getting adequate air movement. If the environment is keeping you from listening, place you’re hands on the chest and feel for bilaterally equal rise and fall of the chest. ———————————————————————— I would personally prefer to skip the intubation attempt and go for a surgical cricothyroidotomy. Orotracheal and nasotracheal intubation may be attempted but be aware that musculoskeletal tissue disruption will likely increase the difficulty of intubation. LMAs and I-Gels can be used to buy you some time to prep for a cric, but they don’t prevent regurgitation and aspiration of vomitus and may increase vomiting unless a paralytic has been administered (which is unlikely due to the fact that we likely haven’t gained vascular access at this point in our assessment). Aus diesem Grund trainieren die Freiwilligen an ihren Dienstabenden , die verschiedensten Szenarien, neben den lästigen Muss der Fahrzeugkontrolle, dem Dienstsport oder den Frühjahrsputz sind solche Übungen für die jungen Kameraden die den Weg zur Freiwilligen Feuerwehr gefunden haben immer ein Highlight, aber nicht nur für sie für alle die daran teilnehmen. Da trifft es sich natürlich gut, wenn man eng mit den anderen Hilfsorganisationen zusammenarbeiten kann und damit die Übung noch mehr bereichert und voneinander lernen kann. Oft zeigt ein anderer Blickwinkel mögliche Fehler oder Dinge die man besser machen kann.
