Sacubitril/valsartan, an angiotensin receptor-neprilysin inhibitor, has been shown to reduce the risk of CV death or heart failure hospitalization and improve symptoms among patients with chronic HFrEF compared to enalapril, the gold standard angiotensin-converting enzyme inhibitor. This paper reviews the current understanding of the effects of sacubitril/valsartan and highlights expected developments over the next 5 years, including potential new indications for use. Exercise has a positive influence on the gut microbiome and the human gut microbiome can exert wide-spread effects on mental and physical health. ⠀ Importantly, brain-gut signaling has been shown to be bi-directional, where not only can gut bacteria influence health and behavior, but psychological states can alter gut health. Bacteria can influence neural, hormonal and immune responses as well as the permeability of both the gut and the blood brain barrier. ⠀ Several bacterial metabolites have been identified as possible mechanisms through which exercise can influence how bacteria communicate to the gut and brain. For example, short chain fatty acids (SCFA, e.g., butyrate, acetate) produced by fermenting bacteria can suppress pro-inflammatory cytokines and interact with regulatory T cells to attenuate colitis. Exercise also alters the ratio of Firmicutes to Bacteroidetes. One study even showed that athletes had higher alpha diversity and a higher relative abundance of 40 different bacterial taxa. ⠀ The combined and independent mechanisms through which exercise influences brain health, the gut microbiome, and their interface have yet to fully understood. More research in this area can lead to exciting discoveries for supplements, training protocols, and exercise as adjuvant theory for intestinal disorders. ⠀ ⠀ ⠀ 😎Cool References: ➡️Mailing, L.
